Please call our main phone line at 602-953-9500 to schedule an appointment with either of our offices, Scottsdale or Glendale. If you are unable to keep your appointment, please notify us 24 hours in advance, and we can schedule a new appointment for you at that time. There are occasions when our normal office schedule may be altered, because our doctors are delayed at the hospital, or an unexpected emergency arises. Your understanding and cooperation is appreciated.

By law, we are providers for all workers' compensation medical benefits, whether or not part of an employer's list of physicians. We accept assignments from Medicare. As specialists providing unique care, most plans allow patients to see us out-of-network. Payments are the responsibility of the patient and are due at the time of service.
